Hymen Repair (Hymenoplasty)

 

The hymen (derived from Hymenaeus, the god of marriage and wedding ceremony in ancient Greek mythology) is a membranous tissue with a thickness of 2 to 3 mm, located 2-3 cm inside the vaginal opening, which partially covers the entrance of the external genitalia in women. This tissue, which can be in a variety of shapes, is usually in the shape of a crescent. It is located at the entrance of the vagina and is surrounded by thin capillaries. Although the hymen membrane is usually thought to be a membrane because of its name but it is not actually a real membrane.

 

It has no known anatomically function. It is thought to prevent the entry of microorganisms into the vagina before the fertility age

 

This tissue loses its integrity during the first sexual intercourse (also called the first night, first vaginal penetration), and consequently small amount of bleeding occurs. The amount of this bleeding may rarely be excessive. The bleeding can be stopped by applying compression with cotton or gauze for 10 to 15 minutes, while observing the area with the help of a mirror. If the bleeding persists despite this, you may need to see a doctor. In some cases, there may be no bleeding at all. The absence of bleeding may be due to the excessive flexibility of the hymen and its structure that allows for sexual intercourse.

 

The hymen may lose its integrity and bleed due cycling, horseback riding, falling from high, masturbation, sexual intercourse, and friction.

 

The hymen takes its shape and the features of its structure during the embryonic period, and 1 out of every 1000 women is born without a hymen.

 

If couples have sexual knowledge before the first intercourse, they will have less fears and prejudices about this issue. One of the most important causes of vaginismus is the fear of thinking that the hymen will tear, bleed and hurt.


 

Hymen Shapes

The hymen has many anatomical variations.

 

Annular (Circular) Hymen:

The hymen is in the form of a ring with a circular hole in the middle. If the hole in the middle is too large, the hymen may remain intact and may not bleed despite sexual penetration. In such a case, it is called “hymen suitable for penetration”. It is colloquially known as “flexible hymen".


 

Crescentic Hymen:

The hymen is crescent-shaped. Its upper part is thinner or hollow, and its posterior part is more prominent.


 

Septate (Divided) Hymen:

In the middle of the hymen, there is an intermediate piece of tissue belonging to it, which divides the opening.


 

Cribriform (Multi-hole) Hymen:

The hymen has more than one hole in the middle. With this appearance, it looks like a “sieve”.


 

Imperforate Hymen (Without Hole):

The hymen has no hole in the middle, and the entrance of the vagina is completely closed. Girls with this type of hymen do not experience menstrual bleeding. The menstrual blood cannot be discharged from the body and accumulates in the vagina behind the hymen. It is a very painful condition that requires opening the hymen surgically.


 

Microperforate Hymen (With a Small Hole):

The hole in the middle of the hymen is very small. It may sometimes require widening the hole with surgical intervention


 

Hymen in Multiparous Women (in those who have given vaginal birth):

In women who have given vaginal birth, the hymen is torn as a result of birth and the remaining parts are called “hymenal caruncles” (hymen residue)".


 

In addition, if the hymen develops excessively thick or inflexible, to an extent that may prevent sexual intercourse, a surgical intervention by a gynecologist may be needed.

 

Hymen Examination (Hymen Examination)

If the patient voluntarily wants to have this examination and to have information about the hymen, the examination is performed. The patient is verbally informed about his condition, and no written or official document is given. Official documents can only be obtained from public hospitals with the decision of the prosecutor's office in judicial cases.

 

The result of the hymen examination is never shared with anyone other than the patient. If the patient is under the age of 18, his parents are informed.

 

Why is Hymen Suture (Hymenoplasty) Performed?

Hymen repair can be performed for many reasons. We doctors do not question why the patient want to have it repaired and we do not have the right to do so. In America, the concept of virginity is not given any importance, but many women consulted us for hymen repair. The reason may be remorse, a boyfriend who crossed the line, or a partner who forced the girl to have sex. Sometimes girls may have the sense that the integrity of their body has been breached, and demand this procedure as a treatment. There may also be women who want this just with intent to relax psychologically. An official document is issued for women who have to undergo a vaginal examination due to medical requirements, and then a repair can be made. The patient may want to forget her past. The authority to make changes on a person's body belongs to that person alone. A woman can have a hymen-related operation, just like having mammoplasty and rhinoplasty.

 

How to Sew the Hymen? What is Hymenoplasty?

The hymen is planted in two ways. With the first method, stitches are placed on the hymen. These sutures maintain their strength for 2-3 days. If intercourse occurs within 2-3 days after hymen implantation, bleeding occurs from the place of these stitches. With this method, the probability of bleeding during sexual intercourse is very high.

 

If more than 3 days pass after the hymen repair procedure, there will be no bleeding as the stitches will dissolve. For this reason, it is also called temporary hymen planting. Temporary hymen suturing is done with local or general anesthesia. The process takes 15 minutes. The patient continues his normal life on the same day. It is very difficult for another doctor to understand the temporary hymen repair.

 

In the second method, a new hymen is created between the vaginal entrance and the hymen. This method is also known as flap method and permanent hymen suturing. This tissue, which is created after the permanent hymen repair process, maintains its strength even after years. However, a doctor's control is required 1 week before the planned sexual intercourse to be sure. It can be understood by another obstetrician that permanent hymen is transplanted.

 

Permanent hymen repair is mostly performed under general anesthesia. The process takes 20-30 minutes. The patient can continue his normal life on the same day. Patients coming from out of town can return in the evening of the same day.

 

While performing permanent hymen repair under general anesthesia, vaginal tightening, labiaplasty and clitoris aesthetic procedures can also be applied in combination.
